The ligation may be briefly described as an outpatient treatment for second-degree internal hemorrhoids wit the help of a rubber band. The history of ligation is quiet old and is linked with the writing of the Hippocrates as they mentioned about a similar process in which they used a thread to tie off hemorrhoids. The process mainly involves the application of a small rubber band to the base of the hemorrhoid, in order to stop the blood supply to the hemorrhoidal mass. Thus the hemorrhoid is expected to shrink and die within a period of 2 to 7 days and come out along with the band to fall off during normal bowel movements. The process has become increasingly popular in the recent times with success rate in between 60 to 80%, as it involves less pain than surgical treatments of hemorrhoids, as well as a shorter recovery period.
The first line is the diagnosis and prescribed medications. This is followed by the positioning of the band. Then the band is applied. This is very complicated process where a protoscope is fist inserted into the anal opening. The hemorrhoid is then grasped by forceps maneuvered into the cylindrical opening of the ligator. The rubber band is then applied when the ligator is pushed up against the base of the hemorrhoid.
One of the basic advantages of the ligation is that it does not require any hospitalization which can decrease the ever burgeoning cost of health care. Another major advantage is that the procedure is virtually pain-free except the Rubber Band Ligation which is usually the first line of treatment for symptomatic hemorrhoids where significantly large numbers of patients undergoing the treatment do complain of pain. After under going the treatment generally patients are able to return to work almost immediately and it is reposted that majority return to work the following day. Ligations can be used for the treatment for almost all the degrees and types of hemorrhoids but it advised not to for the acute and thrombosed hemorrhoids. However there are certain disadvantages also. These mainly comprises of complication of the conditions which may lead to pain, bleeding and Band slippage or breakage. Sometimes the condition may be serious with infection and pelvic sepsis and also there are possible chances of anal fissure and formation of thrombosed hemorrhoids.
